Description

• Perform thorough evaluations of patients in both inpatient and outpatient environments, crafting tailored treatment strategies
• Apply physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function
• Engage in collaborative efforts with nurses, physicians, case managers, and interdisciplinary rehabilitation teams to maximize patient outcomes
• Inform patients and their families about therapeutic aims, the discharge process, and home exercise regimens
• Maintain meticulous documentation of evaluations, treatments, and progress in alignment with facility and regulatory standards
• Contribute to care team discussions and interdisciplinary meetings to guarantee cohesive care

Education

• Doctorate in Physical Therapy (DPT) or comparable qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
• Any specialized certifications (neurological, geriatric, etc.) would be advantageous but are not essential

Skills

• Proven experience is highly preferred
• Adeptness in treating patients with multifaceted medical needs and mobility challenges
• Excellent collaboration and communication abilities
• Proficient in utilizing electronic charting and clinical documentation systems
