Diabetes Educator

Overview Diabetes Educator at Providence Medford- OBGYN Health Center and Providence Medical Group Stewart Meadows. This position is part-time, 24 hours per week and a Day Schedule. Responsibilities Provide education for clients with diabetes of all ages according to the individualized needs of the client. This includes a health and education assessment, development, implementation and evaluation of an education plan, and consideration of psychosocial, economic, and language barriers. Perform duties in a classroom or individual outpatient setting in collaboration with a team of RNs and RDs. Accept professional development and community outreach as an integral part of professional practice. Qualifications Oregon Dietitian License upon hire. National CDR Registered Dietitian - Commission on Dietetic Registration upon hire. Three (3) years of experience in treatment and education of diabetes within the last 2 years. Experience with current adult learning theory in relation to diabetes education. Experience with the clinical management of diabetes. Preferred Qualifications Formal coursework or credentials in principles of adult learning theory and patient-centered learning. Certification in Diabetes Education, or willingness to attain CDE certification upon hire. Experience with diverse ethnic groups.
