Pay: $50.00 - $140.00 per visit Job Type: W-2 Per Diem Medical Specialty: Home Health Work Location: On the road Your Role The RN travels to a patient's home to administer services and helps patients maintain their independence. Duties include: administering at-home IVs, changing dressings, cleaning wounds, in-depth assessments, overseeing patient treatment plans, and updating doctors and the agency on the patient’s health. Documentation and communication are done through Synergy Software. Availability Patients need care 7 days a week. You are required to work at least two—three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Work Area City close to you with a group of patients who need care for that day. Responsibilities Providing nurse visits to home health patients (home visits). Performing initial, comprehensive assessments. Document and upload to Agency Portal. Obtaining and documenting physician orders. Initiating and regularly re-evaluating and revising the plan of care in consultation with the Clinical Team Manager. Coordinating care with the Clinical Team Manager, physician and other members of the home care team; informing them of significant changes in the patient’s condition and needs. Performing re-certifications, resumption of care, transfers, and discharges as requested; completing and submitting all related documentation. Assessing the need for services of other team members (PT, OT, ST, MSW, HHA). Performing Home Health Aide supervisory visits per regulatory requirements. Demonstrating knowledge of regulatory requirements. Position Qualifications 1 year visiting RN for Home Health (required). 1 year nursing experience (required). Current CA RN license (required). BLS from the American Heart Association and/or the American Red Cross (required). NSO nursing license insurance (required). IV/Phlebotomy certified (required). #J-18808-Ljbffr
