Position Details PAY : $50.00 - $140.00 per visit Job Type : W-2 Per Diem Medical Specialty : Home Health Work Location : On the road Responsibilities Providing nurse visits to home health patients. Performing an initial, comprehensive assessment and documenting everything & uploading to Agency Portal. Obtaining and documenting physician orders. In consultation with the assigned Clinical Team Manager, initiating and regularly re‑evaluating and revising the plan of care. Coordinating care with the assigned Clinical Team Manager, physician and other home care team members, informing them of significant changes in the patient’s condition and needs. Performing re‑certifications, resumption of care, transfers, and discharges as requested by the assigned Clinical Team Manager and completing and submitting all related documentation. Assessing the need for services of other team members (PT, OT, ST, MSW, HHA). Performing Home Health Aide supervisory visits per regulatory requirements. Demonstrating knowledge of regulatory requirements. Availability Patients need care 7 days a week. You should be able to work at least two to three days each week (or ten productivity units), one weekend a month, and two holidays per year (one winter and one summer). Qualifications Visiting RN for Home Health: 1 year required. Registered Nurse: 1 year required. Current CA RN License. BLS from the American Heart Association and/or the American Red Cross.
