Case Manager (MA/MSW)

Position Summary The Case Manager (CM) provides services to our Acute adults and/or adolescent patients with psychiatric and chemical dependency needs. As part of a treatment team led by a physician, the CM assesses patient needs, develops individualized plans, and provides group and individualized care. The CM’s work with patients is focused on facilitating return to a level of functioning that allows for community living with support systems in place. Responsibilities Individual and group therapy Psychosocial assessments and treatment planning Child and Family team meeting facilitation Qualifications Education: Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field. License: State licensed in social work or clinical related mental health field; LCSW preferred. Experience: Previous experience in a psychiatric health care facility, with direct experience in family, individual and group therapy, case management, and crisis intervention preferred. Trauma-Focused CBT and DBT experience preferred. Additional Requirements: Must be at least 21 years of age. Location 898 E.
