Case Manager (MA/MSW)

Responsibilities Valle Vista Health System is a 132-bed acute care psychiatric hospital located in Greenwood, IN. The hospital offers inpatient acute care and a variety of outpatient services. It supports local nursing students and other behavioral health professionals. Position Summary The Case Manager (CM) provides services to acute adults and/or adolescent patients with psychiatric and chemical dependency needs. As part of a treatment team led by a physician, the CM assesses patient needs, develops individualized plans and provides group and individualized care. The CM's work focuses on helping patients return to a level of functioning that allows for community living with support systems in place. Job Duties Individual and group therapy Psychosocial assessments and treatment planning Child and Family team meeting facilitation Qualifications Education: Master's Degree from an accredited college or university in Social Work or a clinical related mental health field. License: State licensed in social work or clinical related mental health field; LCSW, preferred. Experience: Previous experience in a psychiatric health care facility, with direct experience in family, individual and group therapy, case management, and crisis intervention, preferred. Trauma-Focused CBT and DBT experience, preferred. Additional Requirements: Must be at least 21 years of age. Location: 898 E.
