Clinical Liaison

LifePoint Health is seeking a Clinical Liaison to connect patients, families, and referral sources with our rehabilitation and post-acute services. In this mission-driven role, you will perform clinical assessments, coordinate referrals and admissions, and educate hospitals, physicians, and community partners on available programs. You will support safe, timely transitions of care, ensuring documentation and authorizations are complete while collaborating with interdisciplinary teams.
