Guaranteed Hours: 40/week
Weekend Requirement: Every 3rd-4th weekend as needed
On-call: After-hours/on-call required

Assignment Details
Daily Census: ~5-20 visits/day
Productivity System: Daily point system (productivity noted ~5.2 points/day)
Mileage/Territory: Typical coverage ~40-80 miles; mileage reimbursed at $0.55/mile

Requirements
- Minimum 1-2 years med-surg or acute care experience
- Minimum 1 year home care experience
- Orthopedic experience required
- Required Certifications: BLS, Phlebotomy
- Key Clinical Skills: Admission/OASIS documentation, wound care/VAC management, IV therapy management (including central ports), phlebotomy, post-op and ortho follow-up, trach and G-tube care/education, patient teaching, basic ADL assessments
- EMR: EPIC

Roles & Responsibilities
- Provide skilled nursing care to adult and geriatric patients in home settings
- Perform assessments and create care plans tailored to patient needs
- Administer medications and therapies including wound care, IV therapy, and post-operative care
- Educate patients and caregivers on health management and home care techniques
- Collaborate with a multidisciplinary team to coordinate care and ensure optimal patient outcomes
- Document care accurately and promptly in the electronic medical record

Compliance / Notes
- COVID-19 vaccine: Not required
- Flu vaccine: Required (medical/religious exemptions allowed)
- RTO: Max 7 days; no RTO allowed the first week; no new RTO after submission
- Fit testing: Facial hair must allow proper testing (may be required to shave)
- Pre-start modules required; must be started promptly and completed per compliance timeline
