Hospice Social Worker LCSW

We seek a highly skilled and experienced Registered Nurse with specialties in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, and Operating Room. The Registered Nurse will provide skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Key duties include patient assessments, medication administration, treatment planning, and collaboration with the interdisciplinary team. Strong communication and collaboration with patients, providers, and team members are essential. A minimum of two years of relevant experience is required. Responsibilities:

• Provide skilled nursing care to patients in various specialties including Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, and Operating Room.
• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Administer medications and treatments as prescribed.
• Collaborate with interdisciplinary team members including physicians, social workers, and certified nursing assistants to ensure optimal patient care.
• Manage case coordination and hospice care services for patients with serious illnesses.
• Maintain accurate and timely documentation of patient care activities.
• Communicate effectively with patients, families, and healthcare providers to support patient needs and care goals.

• Active Registered Nurse license in the state of practice.
• Bachelor's degree in Nursing or related field.
• Minimum of two years of hospital, home health, or hospice care experience.
• Proficient clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration abilities.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and reliable automobile for field work.

• Experience in hospice care or palliative care settings.
• Advanced certifications relevant to specialty areas (e.g., ICU, ER certifications).
• Additional training or certifications in case management or patient advocacy.

• Role may require travel to patient homes and community settings.
• Work involves collaboration with a multidisciplinary team in hospice and home health environments.
• Ability to work effectively in fast-paced and emotionally demanding environments.

• Work schedule may include variable shifts depending on patient needs and care settings.
• May require flexibility for on-call duties or weekend coverage as part of hospice care services.
