Schedule:

• Flexible shifts available Monday through Sunday

• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, medical conditions, and medication histories
• Educate patients regarding medical conditions and medication management
• Identify gaps in care and provide recommendations to address these issues
• Visits are 45 minutes on average, including annual assessments or post-discharge follow-up
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Arkansas
• Board certification (AANP or ANCC)
• Current CPR certification (BLS)
• MSN from an accredited nursing program
• Six months or more of experience as a Nurse Practitioner, preferably in home health
