About the Role: As a Home Health Aide, you will provide support and assistance to patients in their homes, allowing them to maintain their independence and comfort. Your responsibilities will include: Assisting patients with daily living activities, such as bathing, dressing, and grooming Providing emotional support and companionship to patients and their families Assisting with light housekeeping and laundry tasks Helping patients with medication reminders and management Monitoring and reporting changes in patients' conditions to the healthcare team Collaborating with other healthcare professionals to develop and implement patient care plans Requirements: High school diploma or equivalent required 4-6 months of experience as a Home Health Aide (HHA) preferred Strong communication and interpersonal skills Ability to work independently and as part of a team Reliable transportation and valid driver's license (if applicable) Benefits: n/a Why Join SYNERGY HomeCare of Mt.
