Case Manager - Ulster County (Spanish)

Bilingual Case Manager (English/Spanish) Hourly rate: $23.00 per hour. The position comes with benefits. Summary This position provides direct case management services to the needy and disenfranchised in Ulster County. Services include advocacy, information provision, referrals, crisis intervention, and short-term case management. The manager also handles reception duties, answering telephones, and processing clients for services. Essential Duties and Responsibilities Provide crisis intervention to homeless individuals and families facing housing emergencies. Serve as primary case manager for the New York State Rental Supplement Program (RSP), Shelter Arrears Eviction Forestallment program (SAEF), and emergency relief program. Conduct thorough needs assessment and develop a service plan using a short-term intervention model. Perform social care screenings of Medicaid members using the Accountable Health Communities (AHC) tool on the Unite Us platform and refer cases to the Social Care Manager as needed. Navigate individuals not eligible for enhanced services to available local, state, and federal programs. Thoroughly log all client interactions in Unite Us, Apricot, or other required systems. Keep accurate and complete work files, including records, documentation, and other materials. Maintain up-to-date documentation of all activities and services provided to clients. Process cases in a reasonable timeframe, preferably 45 days. Answer telephone and provide information as needed. Conduct outreach to different communities and perform data collection activities. Assist with food/kids pantry distributions. Participate in outreach activities on weekends and after hours such as community tabling, community outreach, and pop‑up pantry events; and engage in special projects like Stuff-The-Bus, Adopt‑A‑Family, Giving Tree, and school supply distributions. Develop and maintain working relationships with providers throughout Ulster County. Attend community or service provider meetings. Be available for planned activities on weekends and some after‑hours. Participate in regular supervisory sessions with the immediate supervisor. Attend the Agency’s in‑service training. Travel to meet clients at other CCOSU offices. Perform other duties as assigned by the supervisor or program demands. Position Type and Hours of Work This is a full‑time position. Typical hours are 9:00AM to 5:00PM, Monday through Friday. Additional hours may be required to meet program deadlines or client needs. Working Conditions and Physical Demands Ability to work with the public and/or under potentially stressful conditions. Basic understanding of client engagement and de‑escalation techniques. Prepare comprehensive progress notes and service plans. Remain stationary at a workstation and use a computer at least 30% of the time. Qualifications Bachelor’s degree in Social Work or related field – Preferred. Associate’s degree with one year of experience in Human Services. Must possess a clean NYS Driver’s License and maintain it throughout employment. Skills, Licenses, and Competencies English/Spanish bilingual – required. Knowledge of social services available in Ulster County – Preferred. Computer literate with Microsoft Word, Excel, and Outlook. Excellent interpersonal and communication skills.
