Patients trust us to receive the right personalized care where and when they need it – in our care centers, at home or virtually – to improve their health outcomes and quality of life. **How will you make an impact & Requirements****Primary duties may include, but are not limited to:*** Provides a range of primary, acute and/or specialty healthcare services for patients in home or when appropriate virtual settings.* In states that do not require collaborative agreements, functions as the primary care provider.* Orders, interprets and evaluates diagnostic tests to identify and assess patient's clinical problems and health care needs.* Provides health instruction and counseling, educating patients on disease prevention and healthy lifestyle choices.* May work in collaboration with other Physicians, Nurses or other healthcare professionals to treat the needs of the patient(s) as part of an interdisciplinary care team.* Formulates and documents care plan.* Prescribes medication or other forms of treatment.* Might participate in On-Call program, which may include after-hours, weekend and holiday calls and visitations.**Requirements:*** Requires an MS in Nursing* Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.* Requires a current, active, valid and unrestricted RN license and NP license in Nevada.* Satisfactory completion of a Tuberculosis test is a requirement for this position.\*\*The posted compensation range represents the national market average.
