PT - Physical Therapist

Outpatient Physical Therapist

This is a 13-week contract position.

5 x 8.0 hours per week

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. The therapist is responsible for the delegation and direction of therapy services for the assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. The role includes serving as an educator for patients, families, students, and coworkers while upholding the organization's Mission Statement and Core Values.

Responsibilities:

• Completes comprehensive evaluations and develops treatment plans focused on goals for patient discharge.
• Documents medical records accurately, including goal writing and assessments per clinic policies and regulations.
• Participates in student programs to provide a positive learning environment through supervision and evaluations.
• Creates and actively pursues professional development plans with clinical management.
• Maintains professional licensure in good standing and adheres to ethical conduct standards.
• Understands and participates in the organization's performance improvement initiatives.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school
• 1+ years of outpatient clinical experience
• Current CPR certification
• Active physical therapist license for the state where the work will be performed
• Previous experience with our agency is highly desired
