Key Responsibilities\n

• Conduct evaluations and assessments to determine the developmental needs of young children in hospice care.

• Participate in the development and implementation of Individualized Family Service Plans (IFSP), ensuring they are personalized and responsive to each family's circumstances.

• Monitor and document outcomes as a member of the multidisciplinary team, regularly assessing progress and adjusting strategies as necessary.

• Provide essential emotional and practical support to families, helping them navigate the complexities of caregiving.

• Implement evidence-based practices and adhere to Natural Learning Environment Principles to optimize each child's learning and development experience.

• Develop rapport with children, their families, and other team members to create a nurturing and collaborative atmosphere.

• Utilize a coaching model to enhance family skills and empower them in supporting their child's development effectively.

• Ensure compliance with all relevant federal, state, and facility regulations, maintaining the highest standards of care and safety for all individuals involved.

Patient Population / Client Focus\n

This role focuses specifically on assisting infants and toddlers with disabilities or delays, particularly as they approach the delicate stages of hospice care. Compensation and Benefits\n

The position offers a competitive annual salary ranging from USD 75,000 to 80,000, commensurate with experience and qualifications.
