Job Description & Requirements Specialty: Primary Care Discipline: Nurse Practitioner Duration: Ongoing Employment Type: Staff Work Schedule: 100% FTE, Clinic schedule, Monday - Friday between the hours of 8:00 AM - 5:00 PM. Call Schedule: shared family medicine call group. Be part of something remarkable As an Advanced Practice Provider, you'll play an integral role in our ambulatory care teams by providing high-quality, cost-effective care to patients We are seeking a Nurse Practitioner OR Physician Assistant to: Be a Primary Care Provider (PCP) Care for the acute and chronic needs of patients from newborn to elderly. Perform annual physical exams and manage the health maintenance needs of our family medicine population. Perform routine clinical procedures independently Education If Nurse Practitioner: Master's or Doctorate's degree from accredited Nursing Program. If Physician Assistant: Bachelor's or Master's degree from accredited PA Program. Work Experience Requires 3 years or 3600 hours of primary care experience Licenses and Certifications Minimum – State of Wisconsin NP or PA licensure If Nurse Practitioner*: Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND Active board certification as a NP in area of clinical practice.
