What You'll Do Evaluate patients in their homes and establish individualized treatment plans. Improve mobility, balance, strength, endurance, and functional independence. Recommend adaptive equipment and home safety modifications when appropriate. Educate patients and caregivers to maximize long-term outcomes. Collaborate with physicians, nurses, occupational therapists, speech therapists, and the interdisciplinary care team. Maintain timely and accurate clinical documentation.

What We're Looking For Active Minnesota Physical Therapist (PT) license. Strong clinical reasoning and patient-centered approach. Excellent communication and relationship-building skills. Ability to work independently while managing a flexible field schedule. Valid driver's license and reliable transportation.
