Genesis Community Homes is looking for a dynamic, hands-on Care Coordinator to drive excellence in healthcare coordination for the individuals we serve, improve staff development, and program quality. If you’re passionate about improving lives and leading from the front, this is your opportunity to shine. What You’ll Own Medical & Care Coordination

• Coordinate all medical appointments, records, and medication tracking for our individuals and ensure follow-through on physician recommendations
• Manage and maintain MARs, Lifetime Medical Reports, Current Medications lists and HRST updates within specified timeframes
• Attend ISP meetings with accurate and up to date information
• Serve as key liaison with providers, pharmacies, specialists and families

Training & Team Development

• Lead Medication Administration & CPR training programs
• Conduct audits, observations, and ensure regulatory compliance
• Partner with leadership to strengthen staff performance
• Design training that builds independence for the individuals we serve

Leadership & Operations

• Serve as a coach and mentor to Residential Supervisors and staff
• Drive compliance with PA ch.6400 & ODP regulations
• Lead program audits and improve systems
• Act as Certified Investigator / Incident Lead
• Help maintain safe, structured, therapeutic homes

What Makes You a Great Fit

• Strong knowledge of IDD, mental health, or TBI populations, 6400 regulations & ODP standards
• Proven leadership and coaching ability
• Highly organized, proactive, and solution-focused
• Comfortable working independently and making decisions
• Tech-savvy (Microsoft 365 Suite, HCSIS/EIM)

Requirements

• Bachelor’s degree in Social Work, Human Services, or a related field, plus 4 years of experience, including 2 years of management experience OR
• Associate’s degree in Social Work, Human Services, or a related field plus 6 years of experience including 2+ years of management experience
• Valid driver’s license
• Willingness to be flexible with your schedule (some evenings/weekends as needed) and be on-call as a resource for residential supervisors
