Responsibilties A day in the life of a Supervisor, Clinical Therapists at Hackensack Meridian Health includes: Hours are: Monday - Friday 7:00 a.m. - 3:30 p.m. Facilitates collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement. Supervises the staff in all care management processes, including LOS, throughput, and patient flow. Applies process improvement methodologies in evaluating team members' documentation. Ensures that all clinicians attend treatment team meetings and team huddles. Provides direction to the multidisciplinary team as needed in complex cases. Obtains, interprets, and presents metrics related to care management, including IMM (Important Message from Medicare) compliance. Attends key meetings and presents information about existing case management processes. Develops the performance improvement plan of the clinical therapists, documents performance and provides performance feedback, evaluates the work of the team member and provides reward and recognition for proper and efficient performance, to include chart audits. Determines areas of opportunity and suggests process improvement. Follows HR policies for performance and disciplinary action. Responsible for disciplinary action and performance improvement plans when appropriate. Participates in departmental preparation for regulatory visits and compliance audits. Coordinates/facilitates patient care progression throughout the continuum by working collaboratively with the multidisciplinary team. Collaborates with ancillary departments to ensure timely discharges. Assists in the collection and reporting of indicators tracking efficiency of clinical therapist processes. Uses data to drive decisions and plan/implement performance improvement strategies related to assigned staff, including fiscal, clinical, and patient satisfaction data. Collaborates with LIPs and providers. Maintains annual competencies and ensures training and continuing education of the team in applicable platforms. Assumes responsibility for supervision of other clinical therapists in the absence of the director. Manages and provides oversight over caseload designation. Will be expected to manage a small caseload in addition to supervisory responsibilities. Assists in preparation, coordination, and evaluation of social work policy and procedure. Assists with interview and orientation of new staff members. Supervises social work/counseling students completing field placement. Has a working knowledge of various community agencies and resources and refers patients when appropriate. Maintains protected patient health information as required by law, State Licensing, TJC, and HMH policy. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior. Qualifications Education, Knowledge, Skills and Abilities Required: Master's degree in Social Work (MSW). Minimum of 5 years of post-advanced-license experience, to include 3 years of full-time experience in an acute care setting. Care coordination, case management, or discharge planning experience. Proficiency with policy review and program planning. Ability to work as part of a team, while building collaborative relationships. Must possess excellent follow-through and attention to detail. Must possess excellent written and verbal communication and presentation skills. Familiar with hospital resources, community resources, and/or resource/utilization management. Effective decision-making /problem-solving skills, demonstration of creativity in problem-solving, and influential leadership skills. Proficient computer skills that include but are not limited to Google Suite and/or Microsoft Office platforms. Licenses And Certifications Required NJ Licensed Clinical Social Worker. Licenses And Certifications Preferred Basic Cardiac Life Support Certificate. Case Manager Certified. Skills: Demonstrated proficiency in relevant skills and competencies. Geographic Location: Cost of living and market rates for the specific location. Internal Equity: Compensation is determined in a manner consistent with compensation ranges for similar roles within the organization. Budget and Grant Funding: Departmental budgets and any grant funding associated with the job position may impact the pay that can be offered. Some jobs may also be eligible for performance-based incentives, bonuses, or commissions not reflected in the starting rate. Certain positions may also be eligible for shift differentials for work performed on evening, night, or weekend shifts.
