Administer prescribed medications and therapies, manage IV therapies, and respond to changes in patient condition with appropriate interventions. Monitor vital signs, track symptoms, and coordinate with the interdisciplinary team to adjust care plans as needed. Document care, interventions, and patient responses in Cerner and Epic electronic medical records; ensure accurate and timely charting. Educate patients and family caregivers on symptom management, safety, and end-of-life care planning; support families through the care trajectory. Adhere to clinical policies and safety protocols (including HLFS requirements) and maintain high standards of infection control and patient safety.
