Director of Nursing - Lake Pointe Rehabilitation & Nursing Center

Location: Conneaut, Ohio

Full-Time • Salary • Immediate Hire

Lake Pointe Rehabilitation & Nursing Center, a 74-bed facility located on the banks of Lake Erie, is seeking an experienced and compassionate Director of Nursing (DON) to lead our nursing department. As a full-time, salaried leadership role , this position offers a comprehensive benefits package .

About the Role:

Our skilled nursing facility in Conneaut, OH is dedicated to providing exceptional long-term care and rehabilitation services. We value strong clinical leadership, teamwork, and a resident-centered approach to healthcare. As the Director of Nursing , you will oversee all clinical and nursing operations within the facility, ensuring high-quality resident care and regulatory compliance.

Responsibilities:

• Oversee all clinical and nursing operations within the facility

• Ensure high-quality resident care and regulatory compliance

• Lead, train, and support nursing staff (RNs, LPNs, STNAs)

• Develop and implement policies, procedures, and care plans

• Collaborate with interdisciplinary team and facility leadership

• Monitor staffing levels, scheduling, and performance evaluations

• Maintain accurate clinical documentation and records

• Promote a positive, team-focused culture within the nursing department

Requirements:

• Active Ohio Registered Nurse (RN) license (required)

• Previous leadership experience in long-term care or skilled nursing (strongly preferred)

• Strong knowledge of state and federal regulations, and experience in managing positive survey outcomes

• Excellent communication, leadership , and organizational skills

• Ability to work full-time hours and manage a nursing team effectively
