Nurse Practitioner

The incumbent provides primary healthcare services for non‑acute or chronic health conditions in specific populations, following applicable scopes and standards of practice and the organization’s policies, values, and mission within the CRNP scope of practice. Work is performed in a physician office or offices dedicated to Primary Care, with weekend and on‑call coverage on a rotational basis as required. Essential Responsibilities Provide appropriate patient care by recording medical history and physical assessment, developing therapeutic care plans, ordering diagnostic tests, interpreting results, and documenting accurately. (25%) Communicate and coordinate patient care with patients, families, community health services, and the interdisciplinary team. Involve attending physicians regarding the plan of care and any clinical decisions outside the CRNP’s knowledge base or scope of practice. (15%) Monitor and ensure continuity of care and coordinate periodic physician evaluations as defined in the written agreement between the CRNP and supervising physician(s). (10%) Prepare written prescription orders for drugs and controlled substances, administer medication and injections, perform sutures, and provide wound care. (10%) Educate patients and family members about health, preventive medicine, prescribed medications, treatment instructions, and recommend community resources as appropriate. (10%) Triage patient telephone calls and provide monitoring to maintain continuity of care. (10%) Participate in, develop, and implement performance and/or quality improvement initiatives. Ensure compliance with all regulatory agencies. (10%) May perform additional duties such as managing outpatient concerns or other specialty‑specific tasks. (10%) Other duties as assigned or requested. Qualifications Required Qualifications (Nurse Practitioner) Master’s degree (MSN) from a State Board of Nursing‑approved Master’s Program. National Board Certification for Nurse Practitioners through AANP, ANCC, NCC, or PNCB. Practicing‑state licensure as a Nurse Practitioner. Prescriptive authority in the practicing state within 60 days of hire. Drug Enforcement Agency licensure within 60 days of hire. CPR certification from the American Heart Association. Criminal background clearance certificates: Act 34, Act 33, and Act 73 (FBI Fingerprinting). Preferred Qualifications None. Required Qualifications (Physical Assistant) Master’s degree in Physician Assistant Studies. Current PA Physician Assistant license. PA‑C or board eligible with ongoing successful recertification (NCCPA) within 90 days of hire. Drug Enforcement Agency licensure within 60 days of hire. CPR certification from the American Heart Association. Act 33, Act 34, and Act 73 criminal background clearance certificates. Substitutions Four to ten years of related experience and/or training, or an equivalent combination of education and experience.
