Job Title Licensed Practical Nurse Grade 29 Department Clinical Services Supervisor/Reports To Clinical/Practice Manager Employment Type Non Exempt (FLSA) Written/Revised Date 2-26-2014 Essential Job Functions Provide professional nursing care to office patients in noncomplex situations under the supervision of Physician and Clinical Practice Manager or Clinical Coordinator. Deliver direct patient care within the scope of licensure and in accordance with SMG policy. Assume accountability for the quality of care delivered, including maintaining standard precautions. Maintain efficient patient flow. Collect lab specimens according to departmental guidelines and accurately label specimens. Prioritize patient care by implementing physician orders and nursing procedures. Document appropriately in the Electronic Health Record (EHR). Assure all patients are correctly identified prior to providing and/or documenting care. Communicate and provide care consistent with age, cultural, spiritual, and developmental needs of the patient. Employ appropriate and timely use of tasking in EHR. Schedule appointments appropriately and accurately according to department guidelines. Administer injections according to appropriate protocol and document injections accurately in EHR. Appropriately demonstrate telephone triage protocols and document in EHR. Effectively communicate problems, concerns or issues to the Office Supervisor/Manager promptly. Ensure proper preparation for patient visit and prepare pertinent information needed for patient visit. Obtain vital signs: BP, temp, pulse, height & weight accurately and completely as required and document appropriately in EHR. Review and update medication list in EHR, demonstrate understanding of prescription control and prescription refill procedures, and accurately and completely document known allergies. Appropriately enter orders and schedule appointments in the Practice Management System (PMS) and EHR. Provide referrals and/or consult orders appropriately and in a timely manner in the PMS and EHR. Facilitate transition of care to UCC/Hospital. General Job Functions Appropriately adhere to universal safety precautions when administering medications and disposal of medical waste. Maintain proficiency with department specific equipment, troubleshoot and report malfunctions promptly. Check and maintain logs for emergency equipment according to policy. Assist in ordering and maintaining all supplies as assigned. Demonstrate flexibility with various work schedules. Recognize and perform duties which need to be performed although not directly assigned. Physical Job Requirements Pushing and pulling, taking frequency and weight into consideration. Education, Certification, Computer and Training Requirements Vocational / Technical School / Diploma Program required. Associate’s degree preferred. New Jersey State Nursing License and BLS certification required. 0 – 1 years experience; 2-4 years experience preferred. Ability to communicate in English, both orally and in writing. Strong interpersonal and organizational skills. Ability to prioritize and organize multiple tasks in a timely manner. Experience with Standard Office Equipment (Phone, Fax, Copy Machine, Scanner, Email/Voice Mail) preferred. Experience with Standard Office Technology in a Windows-based environment preferred. Experience with patient care equipment preferred. Experience with EHR, Practice Management System and Departmental/Ancillary Systems preferred.
