Impactful Intake & Referrals Coordinator

Sevita

Mentor Community Services, part of the Sevita family, seeks an Intake Coordinator to manage referrals, collect demographics, and facilitate placement and funding for individuals in need. You will work with clinical and operations teams to ensure timely, compliant intake and documentation.
