Physical Therapist Professional license in Massachusetts Job Details

Provide physical therapy services to patients in their place of residence who have physical impairments, functional limitations or disabilities resulting from injury, disease, or other causes. Patients may range in age from birth to over 100 years. Collaborate with patient, family, physician, and other allied health professionals to identify patient needs and coordinate services to meet those needs. Utilize thorough and timely electronic documentation and interdisciplinary communications in accordance with agency policies, procedures, and standards of practice of MGH Home Care. Job Requirements

Weekend rotations are once every 6 weeks. Additional Details

MGB Badge Photo required for onboarding: Submit a color photo taken in past 6 months with plain background, no selfies or filters. MGB MVR Check required within 45 days of start for all home care positions. Documentation of MMR vaccination or immunity required. Documentation of TB skin test or blood test within 3 months of start date required and updated if >6 month break. Physician Health Statement within 365 days of start date required and updated if >6 month break. MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break. MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break. MGB Resume including past 7 years of work experience required for submission. 2 references within 4 years required for submission, minimum 1 supervisor or above in skillset. MGB Skills Checklist must be completed within 1 year of submission. MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions. MGB Education Verification through background check company required. MGB SSN Trace must be run with criminal check. MGB National Sex Offender Check run within 30 days of start and updated annually. MGB Exclusion Checks run within 30 days of start and annually. Fit Test (N95) within 365 days to start date and updated annually required. MGB Nurse Aide Registry check within 30 days of start required by MA DPH for all employees. Respirator Medical Clearance Attestation required if fit test is required.
