We are seeking a Full-Time Physical Therapist (PT) to join our dynamic rehabilitation team and help residents regain strength, mobility, and independence.

Physical Therapist Position Overview:

Physical Therapy team members assist with ensuring the health and well-being of individuals by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality care is maintained at all times. State specific Physical Therapy license is required.

Responsibilities:

• Evaluate and develop individualized treatment plans for residents
• Deliver skilled physical therapy services in accordance with physician orders
• Collaborate with interdisciplinary team members to optimize care plans
• Monitor, document, and adjust treatments based on resident progress
• Educate residents and families on therapy goals and outcomes

Qualifications:

• Doctorate or Master's degree in Physical Therapy from an accredited program
• Current Vermont Physical Therapy license (or eligibility)
• Strong clinical, communication, and documentation skills
• Passion for working with geriatric and post-acute populations
