What You Will Do:

• Provide telephonic, virtual (video), and in-person support and guidance to individuals served, including assistance with housing, employment, healthcare, and other essential needsaccompanying individuals to appointments, helping them navigate complex systems, and connecting them with relevant resources.
• Identify individuals in need and evaluate their specific circumstances and challenges through regular weekly outreach and engagement, building trust and rapport to connect them with needed resources.
• Act as a strong advocate for individuals served, ensuring their voices are heard, their needs are met, and empowering them to make informed decisions about their care while promoting autonomy and self-determination.
• Serve as the primary point of contact for all individuals served on caseload.
• Identify individuals served safety issues and take appropriate measures to ensure safety and well-being in consultation with supervisor, program leadership, and/or Emergency Services.
• Provide transportation for individuals served in a personal vehicle as needed when there is a critical need and funder transportation and/or BHN vehicles are not available.
• Include all appropriate parties in the individual's Integrated Care Team, including primary care providers, behavioral health providers, and others as indicated.
• Engage with and refer to Integrated Care Team resources to effectively meet the specialty needs of individuals served.
• Secure appropriate services for individuals served and monitor progress to ensure quality care.
• Build and maintain strong relationships with community partners, including social service agencies, healthcare and behavioral health providers, and other relevant organizations.
• Respond to individuals served requests and make timely referrals to other services and resources as needed.
• Complete and maintain accurate, timely documentation of interactions, progress, and referrals, and complete necessary reports for funding agencies and stakeholders.
• Develop, monitor, and update individualized care plans based on assessed needs, collaborating with individuals served to set goals and coordinating with internal/external providers.
• Comply with program standards for frequency of contacts with individuals served, providers, and collaterals.
• Meet productivity and documentation requirements in accordance with One Care policies and funder contract requirements.
• Work to reduce individuals served utilization of higher levels of care when appropriate and support transitions from inpatient settings back into the community.

Who You Are:

• High School Diploma or GED required; Community Health Worker Certification and/or Bachelor's Degree preferred.
• Bilingual, Spanish and English speaking, preferred.
• 1-2 years of work experience in related behavioral health, human services, and/or medical field required.
• Demonstrates cultural and linguistic competence in working with individuals of varied cultural backgrounds and linguistic needs.
• Demonstrates genuine care and concern for individuals served, acknowledging their struggles and validating their experiences.
• Provides support without judgment, recognizing each individual's situation is unique and complex.
• Pays close attention to individuals' concerns, actively listens, and responds with sensitivity and empathy.
• Treats all individuals served with respect and upholds their dignity throughout the service delivery process.
• Knowledge of and ability to utilize motivational interviewing techniques to help individuals identify strengths, set goals, and develop hope for the future.
• Demonstrates willingness and ability to serve challenging and diverse populations.
• Demonstrates working knowledge of community resources and how to help individuals access them.
• Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance use, and care management.
• Professional demeanor in communicating with individuals served, colleagues, and other providers.
• Ability to communicate clearly and effectively with individuals served, providers, and staff both verbally and in writing.
• Familiarity with Internet, email systems, and electronic health records (EHRs).
• Valid driver's license and reliable, legally registered vehicle available for all working hours required.
• Working knowledge of program KPIs and performance metrics and how they impact the role, the program, and the overall mission of the One Care program.
• Provides exemplary and culturally sensitive customer service to individuals served.

Pay Range: $22.00-$25.40
