Job Title: Case Manager

Profession: Registered Nurse

Specialty: Case Management

Hours per Shift: 8

Experience: Minimum of 3 years

License: Georgia or Compact RN License required

Certifications: BLS

Must-Have: Previous charge experience, Travel experience required

Description: Ability to assess, evaluate and drive discharge plans. Coordinate services to the patient's appropriate level of care. Ensure that the treatment plan is initiated, evaluated, and re-evaluated daily. Follow the progress and lack thereof daily. Monitor length of stay and identify barriers to discharge. Follow regulatory guidelines regarding patient choice. Maintain compliance with all necessary regulations. Meet ongoing assessment requirements as needed. Ensure effective communication across interdisciplinary teams. Utilize clinical criteria for care evaluations. Work collaboratively to enhance patient outcomes. Document all patient interactions and care coordination efforts. Engage in continuous quality improvement initiatives.
