MENTOR Adult Day Health, a member of the Sevita family,  provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

What You'll Do

As an LPN, you will play a vital role in ensuring the health, safety, and well-being of our participants by:

• Providing direct nursing care and health monitoring
• Conducting health assessments and contributing to individualized care plans
• Administering and documenting medications accurately
• Coordinating medical, dental, and specialty appointments
• Serving as a liaison between participants, families, pharmacies, and healthcare providers
• Monitoring medication administration practices and conducting quality assurance reviews
• Responding to participant health concerns and emergency situations
• Maintaining accurate medical records and regulatory compliance
• Training and supporting staff on medication administration, infection control, and safety procedures
• Educating participants on wellness, treatment plans, and self-care practices
• Collaborating with community partners and healthcare professionals to ensure continuity of care
• Supporting participants with daily activities as needed

 

What We're Looking For

• Current Licensed Practical Nurse (LPN) license in Massachusetts
• At least one year of nursing experience preferred
• Experience working with adults, seniors, or individuals with disabilities is a plus
• Current CPR and First Aid certification
• Strong communication, organizational, and clinical skills
• Bilingual candidates, especially those fluent in Cape Verdean Creole, are strongly encouraged to apply
