RN – Home Health
Seven Healthcare are seeking an experienced RN specializing in Home Health for a travel assignment in Beaver Dam, WI. This role involves providing quality care to a primarily geriatric population within a post-acute setting, covering both home health and hospice visits. The successful candidate will have the opportunity to work in an environment that values empathy, compassion, and critical thinking.

Key responsibilities of the RN – Home Health include:
- Provide home care and hospice services for chronic disease management and patient/family education
- Manage a caseload of 5-6 patients per day, including routine and OASIS visits
- Participate in weekly IDT meetings and monthly all staff meetings
- Utilize skills in pain and symptom management, end-of-life care, and complex wound care
- Perform NPWT, ostomy care, central line care, and IV administration
Requirements of the RN – Home Health include:
- Active RN license with travel experience
- Certification in BLS is required
- Proven experience in chronic disease management and hospice care
- Proficiency in Cerner charting system
- Skills in critical thinking and complex wound care management
