- Hours per Week: 40 (guaranteed)/n
- On Call/Weekend Requirement: Yes/n
- Travel: Mileage reimbursed between patient visits/n

/n/nKey Qualifications – Registered Nurse (RN)/n

/n
- Active RN license in the state of Oregon (required)/n
- Current CPR certification (BLS preferred)/n
- Valid drivers license and reliable transportation/n
- Associates Degree in Nursing or higher/n
- Minimum 1 year of nursing experience required (home health experience preferred)/n
- Experience with wound care and HCHB EMR is a plus/n
- Strong clinical assessment, patient education, and care coordination skills/n

/n/nResponsibilities – Home Health Nursing / Community-Based Care/n

- Conduct Start of Care (SOC) visits, recertifications, evaluations, and Resumption of Care (ROC) visits/n
- Provide subsequent skilled nursing visits and ongoing home health nursing care in patients homes/n
- Deliver compassionate, patient-centered care to a diverse community across Portland and surrounding areas/n
- Document all care accurately and promptly using electronic medical records (EMR/HCHB)/n
- Collaborate with interdisciplinary teams, case managers, and physicians to ensure optimal patient outcomes/n
- Participate in on-call and weekend rotations as required/n
