(Physician/MD qualifications required) Family Practice - Family Physician - Blue Island, IL (Telehealth | and Face to Face Visits)-Loan Repayment We are seeking a Family Physician to work for an FQHC in Blue Island, IL both telehealth and face to face. The clinical hours are 8:30am to 5pm Monday through Friday. To see 332 patients monthly, and 3,978 patients annually. Combination of face to face and telehealth visits. The salary range for a New Family Practice Physician: $145,000 $182,000. 3 years or more experience--$ 150,000 to 190,000 annually. Coming in December: Must have been at the facility for a year in order to participate: This FQHC has an incentive plan based on of productivity achievement: 90% or more of monthly visits, Achieving 90% or more of Referrals Report to Interdisciplinary Services within the Health Center, and achieving 90% of closing encounters within 72 hours of visit.
