Patient Followup SpecialistThis employee performs patient followup, generally responsible for the evaluation of patient adherence to physician orders.Job Responsibilities:Provides patient education as an intricate part of the patient care activity, according to the patient need and policyPerforms complete and professional patient assessments in accordance with policies and proceduresMaintains current knowledge of enteral home care techniques and relevant enteral therapy conceptsRepresents the company through professional personal appearance, patient care activities, and content of written reportsGathers necessary information from referral source to ensure insurance coverage of each enteral patient Follows up on necessary paperwork to ensure payment for services renderedMaintains patient records in complete and organized manner in compliance with accreditation standardsDrives to admit, deliver, and pick up such items as enteral formula, feeding pump(s), IV poles, and/or related feeding supplies to patients' home or place of businessProvide patient care, treatment, and service in a continuous and coordinate manner in settings appropriate to meet patient needsPromote safe, effective patient and organizational environments, as well as feeding equipment useParticipate in on-going education that includes in-services, training, and other activities to maintain and improve individual competenceRead and interpret documents such as safety rules, operating and maintenance instructions, and procedure manualsCalculate figures and amounts such as commissions, proportions, percentages, area, circumference, and volumeSolve practical problems and deal with a variety of concrete variables in situations where only limited standardization existsInterpret a variety of instructions furnished in written, oral, diagram, or schedule formShould have knowledge of word processing, spreadsheets, and email communicationsEducation and Experience:One year experience in home health care, hospital, clinic, or nursing home strongly preferredExperience/Education in Dietetics, Nutrition, Home Health, or Nursing strongly preferredOne year of experience as an LPN, DTR, or CNA preferredMust have excellent human relations skills to interface with doctors, other referral sources, and patientsPhysical Demands:The employee must occasionally lift and/or move up to 50 pounds.Lincare offers employees a variety of competitive benefits that include ongoing training and advancement opportunities.
