SUMMARY: 

To provide case management services, counseling, intervention and implementation of Individuals’ plans to assist in developing, maintaining, increasing and maximizing independent functioning in the skill areas identified in the Individual Service Plan (ISP). 

 

OUTCOMES :

1. Ensures that all Individuals have appropriate goals and objectives.
2. Ensures all supporting paperwork is completed and updated.
3. Ensuring all data related to production is accurately recorded.
4. Ensures quality relationships are maintained between referral agencies and The Arc
5. Ensure the safety and well-being of each individual.

1. Be a role model by following Basic Principles and Shift Happens philosophies.

 

AUTHORITIES :

Case Manager of Vocational Services is authorized to:

1. Schedule, conduct and document meetings pertaining to individuals.
2. Train staff on proper documentation. 
3. Assist individuals with learning how to use ADP software to clock in, request time off, approve timecards, and access pay and/or tax documents. 

 

DUTIES:

Tours potential individuals interested in The Arc’s Services and Supports. 

 

Provides new hires training and tools to perform the job. 

 

Ensures Purchase Order of Services, Medical Forms and employment documents are received and correct before an individual begins services. 

 

Coordinates services for assigned caseloads. 

 

Provides consultation as appropriate to assist program staff and individuals in achieving goals and objectives.

 

Creates and implements ISP's.

 

Completes 30 day, Special Staffing meeting quarterly, semi-annual and annual paperwork as required by each program.

 

Provides individual/group information in the areas of vocational, volunteer opportunities, personal and or social adjustment.

 

Monitors and completes Paid Internship Program (PIP) incentive requests accurately and on time with the funding source. 

 

Provides feedback to the supervisor regarding effectiveness of programming and proposing modifications when appropriate.

 

Writes and follows through on implementation of meaningful and attainable objectives to ensure individuals’ growth.

 

Ensures that all forms required by referral sources are completed.

 

Monitors Service Log entries.

 

Monitors billing throughout the month to ensure that Services are being completed correctly and ensures that an individual does not exceed their allotted hours without approval from the referral source. 

 

Provides feedback and 1:1 training to staff on attendance and billing. 

 

Ensures individuals’ payroll is accurate. 

 

Consults as a member of an interdisciplinary team.

 

Ensures all individuals’ records are accurate and up to date.

 

Engages with individuals and staff on a regular basis.

 

Quarterly reporting of Source America contract.

 

Monitors the quality and content of staff documentation as it relates to individuals. 

 

Maintains professional growth by attending in-service training and by reading relevant literature.

 

Conducts semi-monthly in-service meetings with the individuals to review safety protocols, inform them of any changes, distributes payroll checks and answers questions. Ensures individuals attending meetings are signed in for the purpose of Source America compliance requirements. 

 

Supervises and completes documentation of time studies as required.

 

Maintains, updates, and produces electronic reports for individuals’ files. Reports concerns or issues to Program Manager and/or Director of Vocational Services.

 

May be required to provide services on behalf of Vocational Community Specialists in order to meet the needs of the individual and/or program.

 

Distributes medication and follows documentation protocol as written in the Medication Monitory policy.

 

Assists in providing care for individuals with restricted health care conditions. Follows protocol as listed in the Restricted Health condition policy.

 

Completes and/or communicates documentation requirements (i.e., Exit summary, Access to Services, incident reporting, etc.)

 

Performs other duties as assigned to ensure efficient operation of the program and/or agency.

 

SUPERVISORY RESPONSIBILITIES:

May supervise developmentally disabled Individuals. Responsibilities may include training, planning, assigning, and directing work, addressing complaints and resolving problems.

 

QUALIFICATIONS:

EDUCATION and/or EXPERIENCE:

Associate's degree from an accredited college or university; and one to two years related experience and/or training; or equivalent combination of education and experience.

 

LANGUAGE SKILLS:

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, Individuals, customers, and the general public.

 

MATHEMATICAL SKILLS:

Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.

 

REASONING ABILITY:

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of instructions dealing with several variables.

 

CERTIFICATES, LICENSES, REGISTRATIONS: 

Must have reliable transportation, possess and maintain a valid California Driver's License and auto insurance. Standard First Aid and CPR certification, preferred. Certification will be provided within the first 90 days of employment at the expense of the Agency.

 

OTHER SKILLS:

Must be proficient in Microsoft Suite.
