Physical Therapist - Home Health

A Physical Therapist is needed to provide one-on-one therapy care for patients in their home setting. The ideal candidate will assess a person's condition and develop individualized plans of care to help people reach their goals and maximize mobility and independence.

Schedule

Tuesday - Saturday, 8 AM to 5 PM

Responsibilities

• Assess patient conditions and develop individualized care plans.
• Collaborate with an interdisciplinary team to ensure a patient's needs are met.
• Perform standard therapy measurement techniques, including ROM, strength, and balance tests.
• Utilize a variety of therapeutic modalities to maximize care provided to clients.

Required Experience / Certifications / Licensure

• Active Vermont Physical Therapist licensure.
• Home Health experience is a must, along with OASIS experience for case management.
• McKesson/Netsmart EMR experience is strongly preferred.

Assignment Duration

This is a 13-week contract position.
