Join our healthcare partner as a Physical Therapist, where you'll have the chance to work one-on-one with patients in their homes. You'll be responsible for assessing conditions, developing individualized care plans, and collaborating with an interdisciplinary team to ensure comprehensive care. Your expertise in therapy measurement techniques and therapeutic modalities will be crucial in helping clients reach their goals.

• Assess patients' conditions and develop individualized care plans
• Collaborate with interdisciplinary team members to meet patient needs
• Perform standard therapy measurement techniques, including ROM, strength, and balance tests
• Utilize a variety of therapeutic modalities to maximize care
• Case manage using OASIS documentation
• Vermont PT licensure required
• Home Health experience and OASIS experience required
• McKesson/Netsmart EMR experience strongly preferred
• Need 1 year experience in setting
