LPN – Full Time D/E Rotation (Contract)

Location: New York 13165
Pay: $1,306 – $1,474/week
Schedule: Full Time, 5x8-hour shifts (Days/Evenings as needed)
Shift Options: Day and/or Evening rotation
Requirements: Every other weekend & holidays required
Note: Must be able to start on a Tuesday or Thursday for full-shift orientation

Position Summary

• Provide direct and indirect nursing care under the supervision of a Registered Nurse

• Deliver age-appropriate care to adult and geriatric residents

• Support a safe, collaborative, and patient-centered care environment

Key Responsibilities

• Collect and report resident physical and psychosocial data to RN and care team

• Assist in developing, implementing, and updating individualized care plans

• Monitor outcomes and recommend revisions or referrals as needed

• Collaborate with interdisciplinary team, residents, and families

• Maintain clinical competency through continuing education and training

• Participate in staff mentoring and orientation as needed

• Follow safety, infection control, and facility policies

Qualifications

• Active New York LPN License (Required)

• BLS Certification (AHA or RQI only – Required)

• Minimum 1 year LPN experience in a Long-Term Care (LTC) setting

• Experience with adult and geriatric patient populations

• Ability to work in environments with potential exposure to blood/body fluids
