Bilingual Asthma Education & Outreach Specialist
Urban Health Plan
Urban Health Plan in the Bronx is seeking an Asthma Community Health Worker (Bilingual English/Spanish) to support asthma self-management and education services. The ACHW will conduct outreach, home visits, and provide preventive and education services to Bronx residents affected by asthma. Minimum qualifications include High School/GED with 2 years in community health or related work; bilingual English/Spanish required; strong written and verbal communication; proficiency with Word/Excel and #J-18808-Ljbffr Urban Health Plan
