Responsibilities
- Assist in implementing therapy interventions under the supervision of the physical therapist, focusing on mobility, strength, and functional endurance.
- Help patients perform therapeutic exercises and activity-based tasks tailored to individual plans of care.
- Support gait training, transfers, balance activities, and use of assistive devices as directed.
- Monitor patient response during sessions, document progress, and communicate changes to the supervising PT.
- Maintain safety and infection control standards; ensure equipment is clean, calibrated, and used correctly.
- Collaborate with the interdisciplinary team to coordinate care and support discharge planning when appropriate.
- Educate patients and families on home exercise programs and safety strategies as directed.

Requirements
- Prior experience as a Physical Therapy Assistant in LTAC or skilled nursing facility settings preferred.
- Ability to work the assigned daytime shift (08:00-16:30), 5x8 schedule.
- Strong communication and teamwork skills to collaborate with the therapy and nursing teams.
- Able to follow the treatment plan under supervision, document accurately, and manage a varied patient caseload.
- Demonstrated reliability, punctuality, and adherence to safety and privacy practices.
- Comfortable working in a long-term care environment and adapting to patient needs.
