Job Description

The RN Case Manager coordinates management of care and ensures optimum utilization of resources, service delivery, and compliance with external review agencies. This role provides ongoing support and expertise through comprehensive assessment, care planning, plan implementation, and overall evaluation of individual patient needs. The Case Manager enhances the quality of patient management and satisfaction, promoting continuity of care and cost-effectiveness by integrating functions of case management, utilization review, and discharge planning. The incumbent serves as a patient advocate by addressing patient preferences and the holistic needs of the patient.

Responsibilities

• Conduct comprehensive assessments and care planning for individual patients.
• Implement care plans and evaluate patient needs continually.
• Coordinate with interdisciplinary teams to ensure effective resource utilization.
• Advocate for patients in the discharge planning process.
• Ensure compliance with external review agencies and overall quality of care.

Required Experience / Certifications / Licensure

• Must have a current CA RN license.
• Minimum of 2 years of recent RN Case Management experience, preferably in LTAC or STACH settings.
• Experience with MCG and discharge planning across acute care settings.
• Strong collaboration skills within interdisciplinary teams.
• Must have BLS certification from the American Heart Association.
