Case Manager
The Case Manager in this role will be responsible for coordinating patient care and services. They will assess patient needs and develop care plans to ensure optimal healthcare outcomes. The position requires strong communication and organizational skills. The Case Manager will work collaboratively with interdisciplinary teams. They will facilitate patient transitions of care and support discharge planning. Ongoing evaluation of patient progress will be essential. This role is critical in providing support to patients and their families. The ideal candidate will demonstrate a commitment to quality and patient-centered care. Skills-based hiring will be utilized for this position.
