Exciting Opportunity for RN Case Manager Travel Position!

Job Description:


- Coordinate management of care to ensure optimum resource utilization and service delivery.
- Ensure compliance with external review agencies while providing ongoing support and expertise.
- Conduct comprehensive assessments, develop care plans, and implement strategies tailored to individual patient needs.
- Enhance patient management quality and satisfaction, promoting continuity of care and cost-effectiveness.
- Integrate functions of case management, utilization review, and discharge planning for maximum efficiency.
- Advocate for patients, addressing their holistic needs through effective resource utilization and discharge planning.
- Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication.

Position Requirements:


- BLS certification through the American Heart Association (must be attached to submission packet).
- Minimum 2 years of recent RN Case Management experience, particularly in LTAC or STACH settings.
- Proficient in discharge planning across acute care settings and demonstrated strong interdisciplinary collaboration.
- Active CA RN license (must be attached to resume).
- Availability for shifts: Monday - Friday, 8 AM - 4:30 PM.
