Hospice Admissions RN: Care Coordinator & Educator (KC Metro)

True North Consultants, Inc. is seeking a dedicated Hospice Admissions RN for the Kansas City Metro area (east) to join our team on a full-time basis. The nurse will coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and collaborate with the interdisciplinary care team to ensure a smooth transition into hospice services. This role requires Missouri RN licensure, CPR certification, and hospice experience; schedule is Tuesday–Saturday with a #J-18808-Ljbffr True North Consultants, Inc.
