BayCare’s Madonna Ptak Morton Plant Rehabilitation Center , a 5-star CMS-rated facility, is seeking a dedicated Physical Therapist Assistant (PTA) with experience in Skilled Nursing Facilities (SNF) to join our interdisciplinary rehab team. The PTA will assist in delivering high-quality physical therapy services to patients recovering from surgery, illness, or injury in a post-acute inpatient setting. Responsibilities Implement treatment plans developed by the Physical Therapist to improve patients’ mobility, strength, and functional independence. Provide therapeutic exercises, gait training, and use of assistive devices under PT supervision. Monitor patient progress and communicate observations to the supervising Physical Therapist. Document treatments and patient responses in accordance with facility and regulatory standards. Educate patients and caregivers on exercises and techniques to support continued recovery. Collaborate with nursing staff and other rehab professionals to ensure coordinated care. Maintain a safe and clean treatment environment, adhering to infection control protocols.
