Summary This position is located at the Akron VA Clinic Community Based Outpatient Clinic in Akron, OH. Responsibilities VA Careers - Licensed Practical Nurse: Total Rewards of a Allied Health Professional Duties include, but are not limited to: Conducts independent home visits for Veterans with complex medical and psychosocial needs, delivering direct nursing care and providing education to patients and caregivers in alignment with the individualized care plan established by the RN Case Manager. Collaborates with the Home Based Primary Care (HBPC) Interdisciplinary Team including the Physician Preceptor, Registered Nurse, Licensed Practical Nurse, Occupational Therapist/Physical Therapist, Social Work, Dietitian, Speech Therapist, Psychologist, Pharmacist and Advanced Practice Provider. Collaborates with services internal and external to the VA to manage the care needs of the patients. Provides patient and family health education with a focus on self-management, prevention, health promotion and wellness based on the patient's individual goals. Supports additional teams by completing home visits as needed based on patient care demands. Demonstrates strong prioritization skills in determining the order of patient visits according to acuity and need. Utilizes Home Based Primary Care (HBPC) policies and procedures to provide autonomous, high-quality care in the home setting. Possesses the knowledge and skills to carry out more specialized patient care activities. Knowledge of program regulations and is able to carry out objectives of the Nursing program. Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur. Grade Determinations: Must possess at least one year of qualifying experience at the GS-6 level or equivalent. You must fully meet all performance requirements for the GS-6 LPN/LVN or equivalent and your resume must fully support this. Examples of experience include proficiency in initiating, performing and completing assigned duties in providing care to variable patient populations, knowledge and ability to appropriately carry out assigned patient care based on patients' conditions; selecting the appropriate order and sequence of procedures and treatments; completed work only needs a general review by a registered nurse or physician; preparing and administering prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous); recognizing urgent or emergent patient care situations, seeking assistance of the RN and/or MD/DO, and initiating appropriate emergency interventions as directed; and performing duties for complex diagnostic tests and/or specialized practices or procedures. Physical Requirements: This clinical position includes a level of physicality normally associated with direct patient care duties performed by nurses for potentially long periods of time. This may include walking, standing, stooping, bending, sitting, reaching, pushing, pulling and navigating stairs. It may include moderate lifting/carrying, not greater than 35 pounds following Safe Patient Handling and Mobility (SPHM) guidelines.
