divh2Nurse Navigator, Renal Medicine/h2pThe GW Medical Faculty Associates is looking for a qualified Nurse Navigator to provide proficient nursing care for an individual or group of patients with less predictable problems and to provide leadership in the implementation of high-quality nursing care. The position will provide support to the departments of both Renal Medicine and Pulmonary Medicine./ppTo perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Other duties may be assigned. Dependent upon the area of assignment, the incumbent performs all, or a combination of, the following:/pulliUses advanced nursing knowledge and the nursing process to:/liliAssess factors of the patients past and present physical, psychosocial, and cultural baseline, health problems, and external support systems that may impact his/her ability to cope with the current health situation./liliDefine a nursing problem list based on the assessment of the patient, based on the consideration of the medical diagnosis and the patients problems./liliDefine realistic, measurable goals related to each problem./liliDevelop a nursing plan that is directed toward the optimal resolution of nursing diagnoses and prevention of future problems./liliEvaluate and document a patients progress toward goals./liliCollaborates with the appropriate healthcare professionals in implementing a comprehensive plan of care./liliProvide an individualized plan of nursing care that assists the patient in maximizing participation in his/her care./liliCollaborate with a physician to revise the nursing plan of care and/ or goals as necessary./liliImplement appropriate patient education programs to meet individual healthcare needs/liliAssist physicians with day-to-day patient care:/liulliTaking histories and assessing the reason for visits./liliAssist with procedures and treatments/liliProvide patient teaching when necessary/li/ulliRecords nursing care using appropriate documentation./li/ulpMinimum Qualifications:/pulliEducation: An Associates degree in Nursing or an equivalent combination of training and experience/liliLicensure: Current D.C. Licensure as a Registered Nurse or evidence of application for licensure in D.C. and current out-of-state is necessary./li/ulpPhysical Requirements:/pulliSit, walk, reach, bend, or twist for long periods of time in a clinical setting./liliMust be able to lift, carry, push, or pull up to 100 lbs. as part of the role./liliRegularly exposed to healthcare settings that may require personal protective equipment./liliRequires manual dexterity to operate a computer keyboard, calculator, copier machine, and other equipment./li/ulpAbout GW MFA:/ppMFA physicians provide comprehensive patient care, offering one practice for the whole person with 52 medical and surgical specialties.
