Home Care Coordinator ACA Supervisor LPN

The Home Care Coordinator ACA Supervisor LPN provides Adult Care Assistant Supervision and ADHC recordkeeping along with some nursing care to participants under the direct and indirect supervision of the Center Manager or designee. The position requires good interpersonal skills and the ability to communicate effectively at all levels. Performs reassessments of Home Care services for program participants, including the coordination of all durable medical equipment, PCA Services and Nursing Facility Care. Responsible for ensuring the safety of the home environment for the participant.

Responsibilities

Supervision: Makes sure that the job requirements of each Adult Care Assistant are met accordingly to standards. Assists in the coordinating and overseeing PACE Day Health Care Programs with required recordkeeping. Responsible for the coordination with Adult Care Assistants of Basic ADL Care of designated participants. This includes but not limited to bathing dressing, catheter care, basic skin care and incontinent care, toileting processes, and meals and snacks. Responsible for ensuring the Adult Care Assistants actively participate as assigned with the Recreational Therapist by scheduling assignments of activities in the different work areas provided.

Leadership: Using nursing process, delegates care to appropriate personnel; integrates cost effective measures into nursing practice; recognizes unit problems. Takes responsibility for the work schedule of the Adult Care Assistant, including but not limited to, work assignments such as: room assignments, bathing assignments, charting, participant load assignments and recording absenteeism and tardiness and leaving early. Reports any identified needs for the multidisciplinary health care services and alternative levels of care to appropriate personnel in order to ensure optimal continuity and expediency throughout the health care delivery system.

Assessments: Assesses the Homecare needs of the frail elderly population and identifies and develops specific plans of care. Home Visits required for each reassessment and for falls that require a home re-assessment. Also provides a home assessment if a new development causes changes in the home. Assesses the safety of the home environment and whether the participant can be left home alone or must have supervision in the home.

Coordination of Home Care services: Assists in Home Care Visits with Home Care Coordinator MDS/LPN. Coordinates and authorizes all durable medical equipment necessary in the home and will be responsible for the maintenance and upkeep of each item. Assists with Diabetic shoes, home equipment, appropriate seating in wheelchairs and all items related to improvement of their needs for ADLs.

Coordinates the implementation of all home care services which include personal care services to ensure that quality services are provided to meet participant needs. Care Planning: Implements a Care Plan addressing all the home care needs for the participant and safety in the home and community. Actively participates in Interdisciplinary team meetings, presenting the prepared care plan for the Participant/family/caregiver to sign showing their accountability.

Communication: Communicates participant changes to team members. Able to implement crucial conversations with Participants, family members and caregivers regarding care in the home and home safety situations.

Quality: Takes responsibility for documentation; demonstrates active participation in QAPI processes; complies with PACE CENTER expectations to meet staffing demands based on participant care needs. Complies with policies addressing safe working conditions; monitors unsafe working conditions and recognizes inappropriate participant care management, resolves issue/problem and completes written reports. Participates in data collection toward assisting the Registered Nurse in assessing participant physical, psychological, and functional assessment. Able to provide input to the Registered nurse in problems that can result with the participants/clients of ADHC.

Technical Tasks: Documents treatment/procedures, observations, nursing care and participant activities in EMR as deemed necessary. Evaluations: responsible for the written evaluations of all Adult Care Assistants and presentation thereof. Responsible for ordering, or designation thereof, supplies Communication and Collaboration Implementing/Managing Care: Care provided conforms to accepted practice standards; provides correct treatments/procedures and other care as prescribed and according to patient care standards. Demonstrates understanding of age-related characteristics and needs of participants served; explains nursing procedures in appropriate Demonstrates and maintains clinical competency in the delivery of participant care services in accordance with the specific identified needs of PACE participants. Provides basic nursing care to assigned participants.

Provides individualized, appropriate and compassionate participant care in collaboration with team members utilizing a participant-specific plan with treatment and rehabilitative goals for desired outcomes.

On-Call Responsibilities: The PACE Home Care Coordinator MDS LPN participates in the Home Care Department's on-call rotation and is responsible for providing after-hours support to ensure continuous, safe, and coordinated care for PACE participants. On-call duties include, but are not limited to: Managing and coordinating after-hours scheduling needs for Home Care services. Providing clinical support, guidance, and problem resolution for home care staff responding to participant needs. Assisting with participant care needs in the home when required to maintain participant safety and service continuity. Responding to calls during evenings, nights, weekends, and holidays as assigned in the on-call rotation. Communicating relevant updates or issues to the Home Care leadership team to ensure appropriate follow-up. The employee must be able to respond promptly during on-call periods and effectively manage urgent or unexpected care needs to support the well-being of PACE participants.

Other Duties as Assigned: Performs other duties as assigned. Only act within the scope of his or her authority to practice.

Qualifications

Must have 1-year nursing experience working with the frail/elderly population.

Graduate accredited LPN program

LPN current License in state of Louisiana; Current CPR certification, BLS

Able to function well in a team environment. Possess good organizational, communication and supervisory skills. Able to effectively handle difficult and unusual interpersonal situations.
