Job Title: Registered Nurse - Case Management Profession: Nursing Specialty: Case Management Duration: 13 weeks Shift: Days Hours per Shift: 8 hours Experience: Minimum 2 years of inpatient case management experience License: State RN License or compact Certifications: Prefer certification in Case Management, BLS (AHA) Must-Have: - Competent with MCG and InterQual - Ability to work independently - Ability to identify work that needs to be done and follow through to completion without prompting - Ability to work collaboratively with the care team - Ability to complete a comprehensive patient assessment including patient status to drive discharge - Ability to drive stay progression and complete discharge planning to expedite patient transition to the next level of care - Ability to document comprehensively, accurately, and in a timely manner Description: This position requires a minimum of 2 years of experience as an inpatient RN Case Manager. The role involves utilization review and collaborative work with the care team. Completion of comprehensive patient assessments is necessary to drive discharge efficiently. Driving stay progression and completing discharge planning is critical to expedite patient transitions. Accurate and comprehensive documentation in the electronic health record is necessary. The team will cover various units as needed but the role will focus on inpatient case management. It is essential to have experience in hospital utilization management and proficiency in InterQual and MCG. The schedule consists of 8-hour shifts, with participation in weekend rotations. Employees may be assigned primarily to one unit but may need to float to another unit/floor while performing the same inpatient case management duties. Candidates are expected to follow dress code requirements which include business casual attire with a white lab coat or designated scrubs. Training and orientation will include system orientation and role-specific training with peers.
