Registered Nurse (RN) – Home Health

Location: Currituck & Dare Counties, NC

We are seeking a compassionate and experienced Registered Nurse (RN) to join our growing Home Health team. In this role, you'll provide skilled nursing care to patients in their homes while helping them achieve the highest level of independence and quality of life.

If you have experience in home health, are proficient with HomeCare HomeBase (HCHB) and OASIS documentation, and enjoy working independently while making a meaningful impact, we'd love to hear from you.

Schedule

• Monday–Friday, 8:00 AM – 4:30 PM
• Full-time (40 hours guaranteed)
• Rotating on-call schedule
• • 1–2 weekday nights per week
  • Every 3rd weekend

Compensation & Benefits

• Guaranteed 40 hours per week
• Mileage reimbursement: $0.41 per mile

Coverage Area

• Currituck County
• Dare County

Productivity Expectations

• 20–25 patient visits per week

Required Qualifications

• Active North Carolina RN license
• Home Health nursing experience
• HomeCare HomeBase (HCHB) experience
• OASIS documentation experience

Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Provide skilled nursing care and monitor patients' health status in their homes.
• Coordinate care with physicians, therapists, case managers, and other healthcare professionals.
• Complete accurate and timely OASIS assessments and electronic documentation in HCHB.
• Educate patients and caregivers on disease management, medications, and home safety.
• Monitor patient progress and communicate changes in condition to the physician.
• Manage medication administration, reconciliation, and physician orders.
• Maintain accurate nursing documentation and patient records.
• Supervise and support Licensed Practical Nurses (LPNs) and other clinical staff as needed.
• Coordinate follow-up appointments and transitions of care, including hospital admissions and discharges.
• Participate in the rotating on-call schedule to provide after-hours patient support.
• Ensure compliance with agency policies, regulatory requirements, and infection control standards.
