Travel Nurse RN - Case Manager - $2,599 per week in Santa Rosa, CA

TravelNurseSource is working with Malone Healthcare to find a qualified Case Manager RN in Santa Rosa, California, 93012!

Key Responsibilities
Coordinate utilization management, resource management, discharge planning, post-acute care referrals, and care facilitation across the acute care continuum
Oversee management of acute patient populations with a focus on coordinated and integrated care to prevent unnecessary admissions or readmissions
Provide discharge planning coordination and intervention for high-risk patients presenting to the Emergency Department
Promote patient wellness, improved care outcomes, and efficient utilization of health services among patients with complex health needs
Work to minimize denials of payment through proactive case management practices
If floated, assignments will remain within Case Management only
Required Qualifications
Active California RN License (must be verified via Nursys within the last 2 weeks)
Basic Life Support (BLS) certification required - must be AHA or ARC issued
Minimum 2 years of registered nursing experience
Must live at least 50 miles from the facility to be eligible (local candidates may be accepted at a local rate)
7-year work history verification required; candidates must provide accurate work history and supporting documentation as needed
All compliance documents due 10 days prior to start date; labs must be completed within 3-5 days of contract signing
Preferred Experience / Skills
Prior experience in hospital-based case management or care coordination
Familiarity with utilization review and post-acute care referral processes
Experience working with complex or high-risk patient populations
